RESUMO

The systematic and comprehensive introduction and interpretation of the Reporting Checklist of Clinical Practice Guidelines for Chinese Patent Medicines V1.0 is conducive to the presentation of high-quality clinical practice guidelines for Chinese patent medicines (CPMs), thus improving their dissemination and use. The Reporting Checklist of Clinical Practice Guidelines for Chinese Patent Medicines V1.0 specifies the requirements for clear, complete and transparent reporting of the whole process of developing CPMs guidelines, containing 4 domains, 17 topics and 43 items, and involving the basic information, recommendations and methods of guideline development. Especially, it details the items related to the rules and regulations of the use of CPMs in the recommendations, which is helpful for improving the completeness, scientificity and practicality of the reporting of CPMs guideline.

RESUMO

This paper summarized the key points and methods in terms of the establishment of the guideline working group and the management of conflict of interests, trying to provide reference for the development of clinical practice guidelines for Chinese patent medicine (CPM). The establishment of the working group is the first important step for developing CPM guidelines. Considering the characteristics of the clinical practice guidelines for CPM, this study suggests that the three key elements of ‘multidisciplinarity’, ‘clinical relevance’ and ‘geographical representativeness’ should be put focus on when forming the working group. The guideline advisory committee, clinical expert group, evidence systematic evaluation group, secretary group and the external review group should be established. All group members should clarify the conflict of interest, and the process and management method of the conflict of interest should be clearly reported.

RESUMO

The irrational use of Chinese patent medicines (CPM) is becoming more and more prominent, which makes the demand for clinical practice guidelines of CPM gradually increase. In order to make domestic scholars understand the latest developments and existing problems of the CPM guidelines, and promote its development, this paper introduced the concept of CPM guidelines, summarized the characteristics of the two development modes, namely “taking CPM as the key” and “taking disease/syndrome as the key”, and analyzed the current methodological status of developing and reporting CPM guidelines. Based on the existed problems, three suggestions have been put forward to optimize the quality of CPM guidelines, which were clarifying the target users and scope of CPM guidelines, establishing an open and transparent mechanism of the personnel involvement and process steps, and formulating implementable and operable recommendations for the use of CPM.

RESUMO

Persistent postural-perceptual dizziness(PPPD) is the most common chronic vestibular disease, the clinical manifestation is dizziness, unstable and non-rotational dizziness for three months or more. And the symptom is exacerbated by upright posture, active or passive movement, and complex visual stimuli. In addition, PPPD is a functional disease, so routine vestibular function tests and imaging tests are often negative. According to the diagnostic criteria established by the Barany Association, the diagnosis of PPPD often relies on history. This article provides a review of PPPD-related questionnaires.

Objective:To detect the expressions of chemokine receptor 6 (CCR6), CC chemokine ligand 20 (CCL20) E-cadherin and vimentin in tissues of colorectal cancer and their paired liver metastases, and to investigate the possible mechanism of CCR6 in liver metastasis of colorectal cancer.Methods:A total of 62 cases (54 cases of colon cancer and 8 cases of rectal cancer) of primary colorectal adenocarcinoma resection with wax lumps were selected from the First Hospital of Shanxi Medical University and Shanxi Oncology Hospital from 2009 to 2017 with complete data, including 20 samples of colorectal cancer resection with liver metastasis during the same period. The expressions of CCR6, CCL20, E-cadherin and vimentin in colorectal cancer and liver metastases tissues were detected by immunohistochemistry, and the relationships between the expressions of CCR6, E-cadherin and vimentin and the clinicopathological features of patients were analyzed. Logistic multivariate regression was used to analyze the relationship between liver metastasis and clinicopathological features, CCR6, E-cadherin and vimentin. Spearman correlation was used to analyze the correlations between CCR6 and E-cadherin and vimentin.Results:The positive expression rate of CCR6 in colorectal cancer tissues was 66.1% (41/62), 85.0% (17/20) in colorectal cancer with liver metastasis and 70.0% (14/20) in liver metastasis tissues. The positive expression rate of CCL20 in colorectal cancer tissues was 83.9% (52/62), 90.0% (18/20) in colorectal cancer with liver metastasis and 90.0% (18/20) in liver metastasis tissues. The positive expression rate of E-cadherin in colorectal cancer tissues was 67.7% (42/62), 50.0% (10/20) in colorectal cancer with liver metastasis and 65.0% (13/20) in liver metastasis tissues. The positive expression rate of vimentin in colorectal cancer tissues was 79.0% (49/62), 85.0% (17/20) in colorectal cancer with liver metastasis and 90.0% (18/20) in liver metastasis tissues. The expression of CCR6 was closely related to lymph node metastasis ( χ2=11.142, P=0.001), liver metastasis ( χ2=4.694, P=0.030) and TNM stage ( χ2=21.785, P<0.001). E-cadherin was closely related to lymph node metastasis ( χ2=4.694, P=0.030), liver metastasis ( χ2=4.253, P=0.039) and TNM stage ( χ2=7.867, P=0.005). Vimentin was closely related to lymph node metastasis ( χ2=7.293, P=0.007) and TNM stage ( χ2=5.712, P=0.017). CCR6, E-cadherin and vimentin were independent of gender, age, tumor site, tumor size and differentiation degree of colorectal cancer patients (all P>0.05). Logistic regression analysis showed that the expressions of CCR6 ( OR=6.812, 95% CI: 1.206-38.474, P=0.030) and E-cadherin ( OR=0.256, 95% CI: 0.069-0.945, P=0.041) were independent factors affecting the liver metastasis of colorectal cancer. Spearman correlation analysis showed that CCR6 was associated with E-cadherin expression ( r=0.454, P=0.044) and vimentin expression ( r=0.509, P=0.022) in 20 iver metastasis tissues of colorectal cancer. Conclusion:CCR6 may promote colorectal cancer progress and liver metastasis by part of epithelial-mesenchymal transition.

Objective@#To explore influence of the WeChat platform on the compliance of continuous treatment of scar in adult burn patients.@*Methods@#A total of 124 adult burn patients, conforming to the study criteria, admitted in the Department of Burns of our hospital from January 2015 to January 2016 were divided into WeChat group (n=63) and control group (n=61) according to the random number table. Patients in control group only received regular discharging rehabilitation guide, while patients in WeChat group received regular discharging rehabilitation guide and joined WeChat platform after being discharged from hospital. Through pushing rehabilitation plan and rehabilitation related knowledge, organizing support discussion of burn patients, answering the patients′ questions, members of WeChat platform intervention group conducted continuous treatment for 6 months on patients of WeChat group. The compliance of functional exercise of patients in two groups in one week before discharge and 3 and 6 months after discharge was evaluated by using the self-made functional exercise compliance log sheet. The compliance of using anti-scar drug and appliance of patients in two groups in 3 and 6 months after discharge was evaluated by using self-made anti-scar drug and appliance usage log sheets. Data were processed with chi-square test, independent sample t test, non-parametric rank sum test of independent sample, and Wilcoxon rank sum test.@*Results@#(1) The compliance ratio of functional exercise of patients in WeChat group (95.24%, 60/63) was close to 93.44% (57/61) in control group in one week before discharge (χ2=0.188, P>0.05). The compliance ratios of functional exercise of patients in WeChat group were respectively 93.65% (59/63) and 87.30% (55/63) in 3 and 6 months after discharge, which were higher than 68.85% (42/61) and 65.57% (40/61) in control group (χ2=12.615, 8.166, P<0.01). (2) The compliance of using anti-scar drug of patients in WeChat group was significantly better than that in control group in 3 and 6 months after discharge (Z=-4.150, -4.563, P<0.01). (3) The compliance of using anti-scar appliance of patients in WeChat group in 3 and 6 months after discharge was obviously better than that in control group (Z=-4.242, -4.301, P<0.01).@*Conclusions@#By using WeChat platform to provide guide for scar rehabilitation continuous treatment, adult burn patients have better compliance to functional exercise and usage of anti-scar drug and appliance.

RESUMO

Objective To compare the efficacy of Si Wu particles to treat young women with chronic attachment inflammation.Methods70 young women with chronic attachment inflammation in department of gynecology from January 2015-July 2016 were grouped by Lottery method.Two groups were treated with levofloxacin and metronidazole, observation group was another given Si Wu particles.To analyze treatment efficacy between two groups.ResultsAfter treatment, total effective rate of observation group was higher than control group(P<0.05).After treatment, whole blood high-viscosity, whole blood low-viscosity and plasma viscosity of observation group were lower than control group(P<0.05).After the third week of treatment, TCM syndrome points of observation group was lower than control group (P<0.05).Adverse reactions and recurrence rate of observation group were lower than control group, but no statistical significance.ConclusionSi Wu particles can improve efficacy to young women with chronic attachment inflammation.

Objective To elucidate the relationship between LYVE-1 and VEGF-C and their expression in laryngeal squamous cell carcinoma(LSCC),and provide theoretic evidence for the judgement of metastasis and prognosis of LSCC,also for the treatment.Methods An immunohistocbemical analysis was performed to 50 specimens of LSCC with lymphatic endothelial marker LYVE-1.Quantitation of lymphangiogenesis growth factor VEGF-C by RT- PCR was performed to 30 specimens of LSCC.Finally the correlation between LVD and VEGF-C mRNA was analyzed with statistics methods.Results LYVE-1 (+) was observed in all LSCC.The median copy number of VEGF-c mRNA was 4-5-fold higher in LSCC than in adjacent normal tissue.There was correlation between tumor VEGF-C mRNA copy number and intratumoral LVD.Conclusion Lymphatic vessels existed in LSCC.There was correlation between high levels of LVD in LSCC than in normal tissue.The high level of VEGF-C may accelerate the lymphatic metastasis by promoting the proliferation of lymphatic vessel.

Objective To clarity the role of VEGF and its receptors in laryngeal squamous cell carcinoma development and metastasis. Methods The protein and mRNA expression of VEGF and its receptors (Fit-1 and KDR) were examined by Western-blotting and semiquantitative reverse transcriptase polymerase chain reaction analysis in laryngeal squamouse cell carcinoma (20 specimens) and the adjacent non-neoplastic laryngeal tissues (18 specimens). Results The protein and mRNA expression levels of VEGF and its receptors(Fit-1 and KDR) were more abundant in laryngeal squamous cell carcinoma than that in the adjacent non-neoplastic laryngeal tissues (ordedy 4.32±2.21, 2.00±0.91. 1.20±0.55, 0.29~0.31. 2.50±1.69, 0.85±0.28. P<0.01). The expression of VEGF and its receptors(Fit-1 and KDR) in laryngeal squamous cell carcinoma was significantly higher in lymph node positive group than that in lymph node negative group (P<0.01 or P<0.05). Positive correlation was observed between VEGF and KDR in the laryngeal squamouse cell carcinoma and adjacent non-neoplastic laryngeal tissues (P<0.01). There was no correlation between VEGF and Fit-1 expression(P>0.05). Conclusion The intense expression of VEGF, Fit-1 and KDR in laryngeal squamous cell carcinoma provides strong evidence linking VEGF, Fit-1 and KDR expression to the angiogenesis associated with laryngeal squamous cell carcinoma. The positive correlation between VEGF expression and KDR expression clearly demonstrates that KDR is the main signaling receptor for VEGF in laryngeal squamouse cell carcinoma. The VEGF/KDR system plays a functional role in the progression of laryngeal squamous cell carcinoma.

Objective To explore the effect of VEGF-C,VEGFR-3 gene in primary culture in vitro of laryngeal squamous carcinoma tissue by dexamethasone (DEX). Methods Applying the technique of Drimary tissue cuiture in vitro,twelve specimens,which were identified pathologically as laryngeal squamous cell carcinoma,were cultured.After succeeded culture of twelve specimens,the cells were divided in to two groups.One continued to cuhum,another gave 10-7 mol/L of DEX.VEGF-C and VEGFR-3 gene express in 12 cases of cell were examined by reverse transcription polymerase chain reactions(RT-PCR).Results The frequency of VEGF-C(83%) and VEGFR-3(58%) in primary culture in wtro of laryngeal squamous carcinoma tissue were higher than that of the primary culture in vitro of laryngeal squamous carcinoma tissue by dexamethasone (DEX) (25%,8%P<0.05).Conclusion The expressions of VEGF-C and VEGFR-3 in Drimary culture in vitro of laryngeal squamous carcinoma tissue by DEX are decreased.It may provide guide to apply DEX in clinic laryngeal squamous carcinoma chemotherapy and laryngeal squamous carcinoma laryngeal obstruction.

OBJECTIVE@#To investigate the role of CDK2 in the development of DNA heteroploid in laryngeal squamous cell carcinoma (LSCC).@*METHOD@#Fifty specimens of LSCC, twelve specimens of atypical hyperplasia and thirty specimens of vocal cord polyp were obtain during operation. Centrosome were detected by anti tubulin antibodies. Immunohistochemical method was used to analyze the protein expression of CDK2 and gamma-tubulin.@*RESULT@#The positive rate of CDK2 and gamma-tubulin in LSCC were 68.0% (34/50) and 78.0% (10/15), all of them were significant higher than those in the vocal cord polyp (P<0.05). Expression of CDK2 in LSCC was positively association with expression of gamma-tubulin.@*CONCLUSION@#This experiment indicated that overexpression of CDK2 was strongly correlated with abnormal proliferation in LSCC, and it may be a valuable marker for the diagnosis and therapy of LSCC.

OBJECTIVE To observe the submucosal infiltrating extent of laryngeal carcinoma tissue by different location,different T staging and external appearance and to analyze the impact of laryngeal carcinoma submucosal infiltrating extent on surgical margin.METHODS Forty-three laryngeal en bloc specimens coming from 43 cases with laryngeal carcinoma were vertically cut-down along tumor long axis,and were slivered into sections by which laryngeal carcinoma submucosal infiltrating distance was measured under microscope.Laryngeal carcinoma latent submucosal infiltrating extent was compared among different location,different T staging and external appearance.RESULTS Basing on tumor external appearance and infiltrating extent,laryngeal carcinoma was classified into three types:exogenic type,ulcerating infiltrating type and mixed infiltrating type.Of 43 specimens,the three types were 10,8 and 25 cases respectively.There was a significant difference in submucosal infiltrating extent between T1~T2 and T3~T4 laryngeal carcinoma.There was also significant difference in submucosal infiltrating extent between exogenic type and ulcerating infiltrating type or mixed infiltrating type.CONCLUSION The submucosal infiltrating extent of laryngeal carcinoma tissue plays an important role in the surgical margin. As laryngeal carcinoma T stage developing,tumor submucosal infiltrating extent is becoming more extensive.Laryngeal carcinoma submucosal infiltrating extents in ulcerating infiltrating and mixed infiltrating type are more extensive than that in exogenic type.The partial laryngectomy should be prudent to be performed in cases with local advanced ulcerating infiltrating and mixed infiltrating laryngeal carcinoma.
